Diabetes and Endocrine Associates
Pardee Diabetes and Endocrine Associates takes care of patients with diabetes and thyroid issues. We offer a full continuum of care in that we also provide diabetic education to our patients for insulin pumps and blood glucose monitors.
Pardee Diabetes and Endocrine Associates welcomes Jonathan Rappaport, MD in 2015. Dr. Rappaport is a board-certified endocrinologist and earned his medical degree from Louisiana State University, completed a residency in internal medicine at Wake Forest University Baptist Medical Center and a fellowship in endocrinology, metabolism and diabetes at Tulane University. Dr. Rappaport provides specialized diabetes care as well as thyroid disease management and general endocrine disease management. He takes an individualized approach for each patient and enjoys working with a multidisciplinary team to employ the latest treatments and strategies. He is certified in ultrasound-guided biopsy and has been involved in numerous clinical research trials during his career, including evaluating the diabetic effects of various pharmaceutical agents.
Addressing Diabetes and Prediabetes in minority communities
In 2015, Pardee Hospital received a grant from the Office of Minority Health to address diabetes and prediabetes in minority communities within Henderson County. Working with the YMCA of Western North Carolina and Blue Ridge Community Health Services (BRCHS), Pardee will actively provide key services like:
Who can attend a community screening?

What is a diabetes nurse navigator?
If you or someone you know has been identified as having prediabetes or diabetes, our nurse navigator can help. Kelly Ware, BSN, MSN is the diabetes nurse navigator for Pardee Hospital and can recommend community resources, coordinate care among providers and help you apply for charity care and/or Medicaid if you are eligible. She can also provide educational home visits if you are unable to attend a class at BRCHS or the YMCA.
About Kelly Ware, BSN, MSN, Diabetes Nurse Navigator
Ware holds a bachelor's and master's degree in community health nursing. During her 12 years of nursing she has worked in general medicine and medical surgical units in hospital settings, has been a clinical nurse instructor and has served as a nurse on various international missions for Doctors Without Borders.
The Perspectives Program at Pardee meets national standards for diabetes self-management education and offers group and individualized education and guidance by Certified Diabetes Educators (CDE). Several major insurance carriers cover a part of the cost.
