Discharge Planning
Discharge planning begins at the time of admission to the hospital. Each unit at Pardee Hospital has a Care Manager to assist patients and their loved ones. The Care Manager has access to community resources, equipment and facilities to help plan for rehabilitation and recovery.Skilled Nursing & Rehabilitation
Henderson County and the surrounding region offers many top-rated skilled nursing and rehabilitation facilities. Many provide 24-hour a day medical coverage and a complete line of inpatient and outpatient rehabilitation services. Your Care Manager is available to answer your questions and discuss all the available options for skilled nursing and rehabilitation.
Home Care
Your physician may recommend home health care upon discharge from the hospital. These services consist of nursing, therapy and social services. While an important part of the recovery process, home health care does not provide around-the-clock care to patients. It often requires the patient have a caretaker to provide much of the care and supervision of the patient after discharge. If medical equipment is needed, your Care Manager is ready to assist you.
