When your doctor decides you are ready to leave
the hospital, a discharge order will be written.
You may want to arrange
with a family member or
friend to help you when you are discharged.
From the time of admission to the hospital, we begin planning for your discharge. Pardee's discharge planning team consists of a medical social worker and a nurse. This team will assist you or a family member in arranging the continued care that you will need at home. The services needed may include placement in nursing home facilities and rehab centers, home health agencies, hospice programs, physical therapy, medical equipment, meals or sitter. If necessary, this team will also assist in planning for nursing facility placement.
Discharge planning is done with careful consideration of the patient's and family's wishes, input from the doctor and evaluation of available resources in the county. The final choice of services belongs to you and your family. The discharge planning team will be able to provide you with a list of agencies and services available.
