At Pardee, we honor our patients and their rights to make decisions about their own health care. As a patient, you have the right to accept, refuse or withdraw from any treatment, even if you are not physically or mentally able to make these decisions at the time of treatment.
Advance Directives are a way in which you can ensure your decisions about your treatment will be honored. The hospital is required to ask you if you have any Advance Directives. If so, we will need copies of these signed legal documents to place in your medical record.
Examples of Advance Directive documents include:
Living Will - A declaration of a desire for natural death.
Health Care Power of Attorney - Allows you to name a health care agent.
Advance Instruction for Mental Health Treatment - Specification of mental health treatment choices.

If you have an Advance Directive such as a Living Will, please let your nurse know.
