Diabetes and pre-diabetes in minority communities

In 2015, we received a grant from the Office of Minority Health to address diabetes and prediabetes in minority communities within Henderson County. Working with the YMCA of Western North Carolina and Blue Ridge Community Health Services (BRCHS), we provide key services like:

  • Free community screenings

  • Referrals to Pardee's diabetes nurse navigator

  • Educational and wellness classes from YMCA and BRCHS (offered in both English and Spanish)

Who can attend a community screening?

Anyone can attend a community screening, though the program targets lower income, underserved populations. To learn more about the program or to schedule a screening, call Kelly Ware, diabetes nurse navigator, at 828-233-2705.

What is a diabetes nurse navigator?

If you or someone you know has prediabetes or diabetes, our nurse navigator, Kelly Ware, BSN, MSN, can help. Kelly can:

  • Recommend community resources
  • Coordinate care among providers
  • Help you apply for charity care and/or Medicaid if you are eligible
  • Provide educational home visits if you are unable to attend a class at BRCHS or the YMCA
