Diabetes Education Program

Pardee’s Diabetes Education Program offers two opportunities to assist with the education and ongoing needs of Type 1, Type 2, and Gestational diabetes care. Track 1 is recognized and accredited by the American Diabetes Association and is known as the ‘Diabetes Self-Management Education and Support (DSMES)’ program.

This program begins with (2) one-hour individual sessions followed by (8) one-hour group sessions. The second track, known as Medical Nutrition Therapy (MNT), maintains a one-on-one approach with your educator and the number of sessions is determined by insurance; average for most patients is (5) one-hour sessions. 

Both program tracks can help you:

  • Improve hemoglobin A1C levels
  • Reduce cholesterol and blood pressure levels
  • Lessen the risk of complications caused by diabetes
  • Decrease overall health costs
  • Lower risk of hospitalization
  • Adopt healthy lifestyle behaviors such as good nutrition and physical activity

What will be covered during each program?

The DSMES program will focus on seven key behaviors that promote successful self-management of diabetes:

  • Healthy Eating
  • Physical Activity and Exercise
  • Daily Self-Monitoring and Management
  • How and When to Take Prescribed Medication
  • Problem Solving Blood Sugar Levels
  • Reducing Risks and Complications
  • Support and Guidance: MNT is an individually tailored plan that provides nutrition education based on a person’s needs

How do I enroll?

Ask your health care provider for a referral or contact us at 828-698-4598 and we can assist in obtaining a referral from your provider. Check with your insurance plan for detailed information about coverage. Program costs are typically covered, at least partially, by most major insurance carriers.
