Accessing your medical records

To access your medical records, print and fill out a Release of Information form. Fax a copy to 828-696-1097 or submit your form in person to the Health Information Department on the ground floor of Pardee Hospital. This office is open Monday through Friday, 8am to 4:30pm.

Requesting access to a family member's medical records

Both patients and families can request medical records by filling out and signing a Release of Information form. Families cannot request records for a patient without written approval from the patient.

A health care power of attorney is only valid if the patient is incapacitated mentally or physically and there is a written note from the doctor in the patient’s chart stating such.
